For the server: The server is most commonly down because it was accidentally unplugged from the power and/or Internet.

For card: The card is most commonly down because the card reader is not properly plugged into the server. Another reason could be that the card is not properly inserted or it was accidentally removed from the card reader. Also, if the card is expired it may show 'Down' status as well.
