To make changes or delete an existing form template, follow these steps:
Go to
'Forms' module and switch to 'Edit forms' tab.
Find the form you want, then either:
Click the pencil icon
next to the form, edit it, and then press SAVE.
Or click the trash bin icon
to delete it, and press "Yes, delete" confirm.
